How do we know if there is a fake keypad on the atm machine? Should we just try to pull the keypads everytime we use the atm?

I think if you use the number on the debit card, you need the generated password to make it through and even if you get the atm password, you still need the card right? But i still don't know about swipe cards though, do you still need the generate code too?
